ABSTRACT

Objective:To analyze the influencing factors of post stroke cognitive impairment (PSCI) and their correlation with cognitive scores in patients with acute ischemic stroke. Methods:In this cross-section study, 36 patients diagnosed with acute ischemic stroke and post stroke cognitive impairment (PSCI) admitted to the Department of Vascular Neurology of Beijing Tiantian Hospital Affiliated to Capital Medical University from June 1, 2022 to September 30, 2022 were selected as the PSCI group. And one to one matching was performed for patients without PSCI (PSNCI group) with an age±1 year and same gender admitted to the hospital during the same period (as control, 36 cases). Basic clinical data of the two groups were collected, the laboratory and imaging examinations were completed. Mini Mental State Examination (MMSE) and Montreal Cognitive Assessment Scale (MoCA) were used for cognitive evaluation by neuropsychologists. Hypothesis testing was used to compare the differences in basic data, laboratory tests and lesion sites between the two groups. Multi-factor conditional logistic regression was performed to analyze the influencing factors of PSCI, and Spearman correlation analysis was carried out to analyze the correlation between influencing factors of PSCI and the cognitive scores. Results:Compared with those in PSNCI group, the proportion of patients with stroke/transient ischemic attack history, hyperhomocysteinemia (HHcy), apolipoprotein E(ApoE) ε4 carriers and the ratio of temporal lobe and thalamus infarction were higher in PSCI group (41.7% vs 13.9%, 36.1% vs 2.8%, 30.6% vs 5.6%, 22.3% vs 2.8%, 25.0% vs 5.6%), the MMSE and MoCA scores were lower in PSCI group [16.50 (8.25, 19.00) vs 28.00 (27.00, 30.00), 10.00 (4.25, 14.50) vs 27.00 (25.00, 28.00)] (all P<0.05). Logistic regression analysis showed that HHcy was a positive correlation factor for PSCI ( OR=2.342, 95% CI=1.186-4.622, P=0.014). Spearman correlation analysis showed that MMSE ( r=-0.415) and MoCA ( r=-0.417) scores were negatively correlated with homocysteine (Hcy) (both P<0.05). Conclusion:HHcy is an important factor affecting the occurrence and development of PSCI in patients with acute ischemic stroke, and Hcy level is negatively correlated with cognitive scores in those patients.

ABSTRACT

Objective:To analyze and compare rehabilitation research funded by China′s National Natural Science Foundation (NSFC) with that funded by America′s National Institutes of Health (NIH) so as to provide references for future funding. Methods:Articles reporting rehabilitation research funded by the NSFC and the NIH were retrieved from the NSFC′s Science Output Service website and the NIH′s Project Report website and analyzed. Results:From 2009 to 2018 the NSFC funded 421 rehabilitation studies which resulted in a published report while the NIH funded 312. In 2018, the NSFC budget (US$3.89 million) was 8.46 times that of 2009 (US$460, 000), while the NIH′s grant budget (US$36.08 million) was 2.17 times that of 2009 (US$16.62 million). The number of published papers resulting from the Chinese and American studies was 1111 and 2571 respectively. Their impact factors mainly ranged between 0 and 3 points. Among the journals with an impact factor of 6 or more, published papers from the United States (297) were much more numerous than those from China (18). The number of SCI papers per million US dollars increased by 2.25 times in China and 0.05 times in the US.Conclusions:Both China and the United States have been investing more in rehabilitation medicine research, and that has resulted in more published papers. There is still a gap in funding and output between the two countries.

ABSTRACT

To investigate the present situation of empowerment in elderly patients with hypertension in the community, and to analyze its impact factors. Methods 176 elderly hypertensive patients in our community of Guangzhou city were investigated by convenience sampling method. The chronic disease empowerment scale, general self-efficacy scale and social support rating scale were used to investigate. Results The score of empowerment in elderly patients with hypertension was (147. 48 ± 15. 65). The score of self-management is the lowest dimension with the score rate of 44. 6%. The impact effects of education level, living conditions, course of disease, self-efficacy, objective support, subjective support and utilization of social support on empowerment were significant(P<0. 05), which could explain 78. 1% of the variance. Conclusion The empowerment level of elderly patients with hypertension in community is relatively low, the clinical medical staff should focus on the patients of lower education level, living alone, with a longer duration of disease. Nurses should pay attention to strengthening self-efficacy and social support of the patients so as too improve the clinical therapeutic effects of patients.

ABSTRACT

BACKGROUND@#Few studies have attempted to compare the differences in the prevalence and impact factors of hysterical tendencies (HTs) in adolescents. Thus, the aim of this study was to examine gender differences in the prevalence and impact factors of adolescents' HTs across three eastern Chinese provinces (Anhui, Jiangsu, and Zhejiang).@*METHODS@#A multicenter, school-based, cross-sectional study was conducted in three provinces (Anhui, Jiangsu, and Zhejiang) in China in 2014. The sample included 10,131 middle-school students aged 13-18 years who were randomly selected using a multiphase, stratified, cluster sampling technique. A two-stage appraisal procedure was used to determine the adolescents' HTs. We also designed a multicenter, school-based, case control (1329 cases with 2661 control individuals) study to collect data on the common factors affecting this population using a common protocol and questionnaire.@*RESULTS@#An overall positive rate of HTs among adolescents across the three eastern Chinese provinces studied was found at 13.1% (95% confidence interval (CI) 12.5-13.8%), at 14.5% (95% CI 13.3-15.7%) for females, and at 12.2% (95% CI 11.1-13.4%) for males. Gender-stratified, multiple conditional regression analyses revealed that superstitious beliefs pertaining to life, somatotype, teacher-student satisfaction, and family achievement orientation were significantly linked to HTs only in males, while left-behind adolescents, emotional and social adaptation, teacher-student support, family cohesion, and the Hospital Anxiety and Depression Scale - depression scores were significantly associated with female HTs only. The models indicated that of all the independent variables studied, family medical history was the strongest impact factor for both male HTs (adjusted matched odds ratio (amOR) = 2.92, 95% CI = 1.84-4.86) and female HTs (amOR = 2.74, 95% CI = 1.59-4.98).@*CONCLUSIONS@#HTs are prevalent among adolescents in the three eastern Chinese provinces studied. Gender differences in the prevalence and impact factors of HTs are significant in adolescents, and HTs seem to affect more females than males. Therefore, sex-specific intervention programs against HTs in adolescents should be considered to reduce HT prevalence in adolescents by modifying influential social, school, and family factors.

ABSTRACT

Objective To study the impacts of pre-pregnancy maternal BMI and gestational weight gain(GWG)on pregnancy outcomes. Methods We adopted a prospective cohort study with cluster sampling in single pregnant women, who were not with hypertension, diabetes, hyperlipidemia or other diseases in the previous history, neither did they have diseases of heart, liver, kidney, thyroid etc. related to current pregnancy. Those pregnant women who visited the prenatal nutrition clinic under‘informed consent’were surveyed with questionnaire to track their peri-natal complications, delivery mode and neonate birth outcomes etc. Pearson and partial correlations,chi-square test and binary logistic regression were used to study the association between pre-pregnancy maternal BMI,GWG and pregnancy outcomes. Results A total of 623 pregnant women were recruited in the cohort, with 592(95%)of them eligible for analysis. Results from the Multivariate Logistic Regression analysis indicated that, after controlling the potential confounding factors, when compared to women with pre-pregnancy BMI between 18.5 and 24.0,the odds ratios(ORs)for low birth ponderal index(PI) were 2.34[95%confidence interval(CI),1.24-4.42)]among those with BMI<18.5,respectively,while 2.73(1.12-6.68)for high birth PI among those with BMI>24.0. Similarly, when compared to pregnant women with normal GWG(defined as weight gain range from P15 to P85 by stratification of pre-pregnancy BMI),low GWG(<P15)seemed to be the risk factor for preterm birth, low birth weight, gestational diabetes mellitus, with low birth PI [ORs as 4.85(1.35-17.51),10.30 (2.29-46.35),2.29(1.07-4.93) and 2.65(1.24-5.68),respectively. High GWG(>P85)appeared the risk factor for high birth weight, high birth PI,and gestational diabetes mellitus, with ORs as 3.83(1.74-8.44),2.39(1.14-5.01) and 2.21(1.07-4.55),respectively. Conclusion Low or high pre-pregnancy maternal BMI and GWG were associated with adverse pregnancy outcomes.

ABSTRACT

The advent of online publishing greatly facilitates the dissemination of scientific results. This revolution might have led to the untimely death of many traditional publishing companies, since today’s scientists are perfectly capable of writing, formatting and uploading files to appropriate websites that can be consulted by colleagues and the general public alike. They also have the intellectual resources to criticize each other and organize an anonymous peer review system. The Open Access approach appears promising in this respect, but we cannot ignore that it is fraught with editorial and economic problems. A few powerful publishing companies not only managed to survive, but also rake up considerable profits. Moreover, they succeeded in becoming influential ‘trendsetters’ since they decide which papers deserve to be published. To make money, one must set novel trends, like Christian Dior or Levi’s in fashion, and open new markets, for example in Asia. In doing so, the publishers tend to supplant both national and transnational funding agencies in defining science policy. In many cases, these agencies tend simply to adopt the commercial criteria defined by the journals, forever eager to improve their impact factors. It is not obvious that the publishers of scientific journals, the editorial boards that they appoint, or the people who sift through the vast numbers of papers submitted to a handful of ‘top’ journals are endowed with sufficient insight to set the trends of future science. It seems even less obvious that funding agencies should blindly follow the fashion trends set by the publishers. The perverse relationships between private publishers and public funding agencies may have a toxic effect on science policy.

ABSTRACT

PURPOSE: The purpose of this study was to analyze recent citation trends in an attempt to improve the impact factor (IF) of the Journal of the Korean Society of Emergency Medicine (JKEM). Data from 2001~2012 in the Korean Medical Citation Index (KoMCI) was compared with the average citation data of other Korean emergency medicine-related journals. METHODS: All citation data from all journals listed in the KoMCI and the Web of Science were obtained. The chronological changes in the annual number of published articles and reference citations, total citations and self-citations per paper, IF and impact factor excluding self-citations (ZIF) were described and compared for Korean EM-related journals and the JKEM. RESULTS: The annual number of articles published in JKEM was significantly larger than other EM-related journals. The number of Korean journal references per article is 2.13 papers on JKEM. The mean IF of the KoMCI was 0.153 and the ZIF was 0.050 for 12 years. The annual IF (ZIF) of JKEM has gradually decreased from 0.245(0.102) in 2010 to 0.118(0.035) in 2012. However, other EM-related journals have shown increases in these factors since 2009. The IF calculated from the Web of Science was zero until 2010, but soared from 0.008 to 0.016 from 2011 to 2012, respectively. CONCLUSION: The citation status of JKEM has steadily decreased over the past 2 years. To make JKEM as a highly-cited journal, an awareness of the academic status of JKEM and active advertising from journal members on the importance of the IF are needed to encourage the citation of its papers.

ABSTRACT

Objective To analyze the hospitalization expenses of cancer patients covered with byitem payment and quota payment packages, and probe into the impacts on such expenses for the two payment packages. Methods Inpatient records of 600 cancer patients were sampled by random from the medical insurance databases of Zhengzhou and Fuzhou to learn their hospitalization expenses and impact factors. Results Under the by-item payment package, the expenses of urban workers’ medical insurance were found higher than those of urban residents' medical insurance, with a per capita expense of RMB 32747.70 ± 32035.01 and 23035.83 ± 22875.65 respectively.Under the quota payment package however, there were no significant differences between expenses of the two kinds of inpatients, with a per capita expense of RMB 66043.41±47562.09 and 66576.54±73417.29 respectively. Conclusion There are gaps of reimbursement level between the two basic insurance schemes, which may not disappear in a short time.Under the by-item payment package, the gap exists in the difference of perreimbursement amount; under the quota payment package, the gap is negligible between the two populations under different insurance schemes. It is recommended to make reasonable use of these different payment schemes to minimize the relative gaps in medical service accessibility caused by the difference in reimbursement level.

ABSTRACT

OBJECTIVE: Impact Factors (IF) are widely used surrogates to evaluate single articles, in spite of known shortcomings imposed by cite distribution skewness. We quantify this asymmetry and propose a simple computer-based procedure for evaluating individual articles. METHOD: (a) Analysis of symmetry. Journals clustered around nine Impact Factor points were selected from the medical ‘‘Subject Categories’’ in Journal Citation Reports 2010. Citable items published in 2008 were retrieved and ranked by granted citations over the Jan/2008 - Jun/2011 period. Frequency distribution of cites, normalized cumulative cites and absolute cites/decile were determined for each journal cluster. (b) Positive Predictive Value. Three arbitrarily established evaluation classes were generated: LOW (1.3

ABSTRACT

PURPOSE: The total number of times a paper is cited, also known as the impact factor (IF) of a medical journal, is widely implied in evaluating the quality of a research paper. We evaluated the citation index data as an IF of Korean J Pediatr in Korean Medical Citation Index (KoMCI) and JCI of Web of Science. METHODS: We calculated the IF of Korean J Pediatr at KoMCI supervised by Korean Association of Medical Journal Editors. And we estimated the IF of Korean J Pediatr by the JCI of Web of Science although it was never officially reported. RESULTS: The IF of Korean J Pediatr on KoMCI has increased from 0.100 in the year 2000, to 0.205 in 2008, and 0.326 in 2009. Although the IF of Korean J Pediatr was 0.006 in 2005, 0.018 in 2006, 0.028 in 2008, 0.066 in 2009, and 0.018 in 2010 according to the JCI of Web of Science, the number of citations are steadily increasing. CONCLUSION: Understanding and realizing the current status will be a stepping stone for further improvement. The next objective of the Korean J Pediatr is to become registered in the SCI or SCIE. Increasing the IF according to the JCI of Web of Science is crucial in order to achieve this goal.
